Echocardiogram of Chronic Aortic and Mitral Insufficiency.



Abstract
Echocardiographic observation on the 15 cases of chronic aortic insufficiency and 8 cases of chronic mitral insufficiency was performed. The M-mode echocardiogram of left ventricle, mitral valve, aortic root -and left atrium of each patient in his resting state was recorded by standard method simultaneously with ECG, and carotid pulse tracing, and from which specific echocardiographic finding of both diseases were observed and various measurements from left ventricle, mitral valve, aortic root and left atrium in conjunction with ECG and carotid pulse tracing. The indices of left ventricular performance were also observed. Characteristic findings of each disease were described and the differential findings between two diseases were discussed; with aortic insufficiency, fluttering of the anterior leaflet of the opened mitral valve(AMV) was observed in all cases, fluttering of posterior, mitral leaflet(PMV) in 7 out of 15 cases, and fluttering of the interventricular septum(IVS) in 4 out of 15 cases; with mitral insufficiency the fluttering of the AMV was observed in one, and fluttering of PMV in 2, and, fluttering of IVS in one out of 8 cases. Dilatation of left ventricle (LV), left atrium(LA), increased IVS/LVPW thickeness ratio, amplitude of systolic IVS motion, stroke volume and cardiac index were observed in both disease groups. But the increased EPSS (the distance from E point of mitral valve echogram to interventricular septum), left ventricular outflow tract width and PR-AC difference( difference between PR interval from ECG and A to C interval from mitral valve echogram) and dilated aortic root were unique findings of aortic insufficiency, whereas marked increase of LAD/AOD ratio (left atrial dimension to aortic root dimension ratio) was pertaining to mitral insufficiency.
